The Wisconsin Immunization Registry is a database used by state and local agencies to track vaccination status across different school districts, counties, cities and other municipalities.

Do you want your information removed from the WIR (Wisconsin Immunization Registry)? Do you want your child’s information removed?

They now have the opt out form online to mail in.

The opt out form is here

Once your vaccination records are LOCKED, the information in the WIR CANNOT be accessed by anyone - NOR can it be used for reporting or tracking purposes.

Locked records won't be deleted but they can't be accessed. If you change your mind, you can contact DHS and have the records unlocked again.

Marge Grant

In 1961, Wisconsin residents Jim and Marge Grant took their infant son Scott in for a routine well-baby check-up. Afterwards, Scottie – as he is known – suffered irreversible brain damage stemming from the DTP vaccine, Quadrigen, which contained the antigens from diphtheria, tetanus, pertussis, and polio organisms (in addition to other ingredients used to manufacture vaccines). Tragically, after vaccination, Scott Grant became a permanently non-ambulatory, spastic quadriplegic.

Physicians at Mayo Clinic, including esteemed pediatric neurologist J. Gordon Millichap, examined Scott in 1962 and confirmed that the vaccines Scottie received at four months, five months, and six months of age directly caused his permanent brain damage which resulted in severe motor and intellectual disabilities. For Scott’s mother Marge Grant, this tragedy led to her life’s mission. She became a lifelong national pioneer for vaccine safety and parental choice and co-founded Wisconsin Citizens for Free Choice in Immunization. Her organization successfully petitioned our Wisconsin state legislature, and in 1980 the Wisconsin legislature voted to allow personal, religious, and medical exemptions to vaccination, for school entry.

We are dedicated to preserving the law which Marge Grant, the parent of a severely vaccine-injured child, fought so valiantly to obtain for us in Wisconsin. Jim and Marge are no longer with us, but their youngest son Scottie remains a resident of Beaver Dam. He is 58 years old and requires 24-hour care due to the injury he sustained from the vaccines he received as an infant.
